What Happens If Your Visiting Application Gets Denied (How to Request an Administrative Review)
A denial doesn't have to be final. In Oregon DOC, either the adult in custody (AIC) or the prospective visitor can request an administrative review.
The timing matters: your administrative review request (CD 1594) has to be received by Visiting Services within 60 days of the date on the visiting application denial letter. Don’t count from the day you opened the mail - use the date printed on the denial letter and work from there so you don’t miss the deadline.
You'll need the Administrative Review form, CD 1594. This is the form Oregon DOC requires for challenging a visiting application denial.
- ✓ Fill out CD 1594 completely
- ✓ Attach all documents you want considered with your request
Send your completed form to: Visitor Services, 3723 Fairview Industrial Drive SE STE 200, Salem, OR 97302-4975.
Practical Tips After Send
- ✓ Make a copy (or take clear photos) of your completed CD 1594 and every document you attach
- ✓ Write down the date on the denial letter and calculate your 60-day “must be received” deadline from that date
Your request must arrive at Visiting Services within the deadline, so I recommend using certified mail or another method that provides proof of delivery. Follow up to confirm they received it at Visitor Services, 3723 Fairview Industrial Drive SE STE 200, Salem, OR 97302-4975.
