1. Ask your loved one to start the request - significant-relationship visits have to be initiated by the offender.
2. Have the offender complete the Significant Relationship Visitor Application for Visitation Privilege - this is the form used for non-immediate-family visitors.
3. Wait for Warden/Superintendent review - the application is sent to facility leadership for a decision.

1. Open the “Cancel my visit” tool - this is the official way to cancel a scheduled visitation.
2. Find your scheduled visitation - locate the exact appointment you need to cancel.
3. Follow the prompts to cancel and save confirmation - keep proof of the cancellation for your records.

After you submit your Visitation Request Form, the facility reviews it before granting approval. Significant relationship applications go through an additional step - the Warden/Superintendent reviews those requests after your loved one initiates them. Whitworth's visitation hours are 9:00 AM to 3:00 PM. Days alternate by last name: Saturdays for M–Z, Sundays for A–L.
