Checklist

  • Your original, acceptable photo ID for check-in, plus a legible copy of your photo ID to include with your visitation application (DRC-2096)
  • Any required custody paperwork if it applies to your situation
  • If you are bringing a minor: a copy of the child’s birth certificate or proof of legal guardianship (include it with the minor’s visitation application)
  • If a minor is visiting without the custodial parent or legal guardian: a notarized Authorization for Minor Child Visitation form
  • If you are on probation or parole: a letter from your probation/parole officer granting permission to visit

Where to send your paperwork: Submit the completed visitation application packet and a copy of your ID directly to the Dayton facility (by mail, fax, or email). Do not send it to ODRC central mail processing.

Photo ID plays a role twice: once during the application process and again on visit day. When you submit the visitation application (DRC-2096), include a legible copy of your photo ID in the packet. Then bring the original with you when you visit so you can check in.

Heads up: If you show up without the required documents (including ID), you will not be permitted to enter Dayton Correctional.

Bringing a child? The paperwork has to match the child, not just the adult. The minor's visitation application needs to include a copy of the child's birth certificate or proof of legal guardianship.

Things get stricter when the custodial parent or legal guardian isn't the one bringing the child. In that case, you must provide a notarized Authorization for Minor Child Visitation form for the Dayton Correctional visit.

  • Copy of the child’s birth certificate or proof of legal guardianship (submit it with the child’s visitation application)
  • Notarized Authorization for Minor Child Visitation form if the custodial parent or legal guardian will not be present

Currently on probation or parole? Your application packet needs one extra document. Include a letter from your probation/parole officer granting permission to visit Dayton Correctional, and keep a copy on hand when you go.

Staff at Dayton Correctional check for the required documents at check-in. If you're missing something for your situation (ID, minor visitation paperwork, etc.), you will not be permitted to enter.

  1. Match your ID to your application: Bring your original photo ID for check-in, and include a legible copy of that photo ID with your visitation application (DRC-2096).
  2. Handle minor visits the right way: If the child is visiting without the custodial parent or legal guardian, make sure you have a notarized Authorization for Minor Child Visitation form.
  3. Include the child’s proof documents: Submit a copy of the child’s birth certificate or proof of legal guardianship with the minor child’s visitation application.

Reminder: Send the completed visitation application packet and a copy of your ID directly to the Dayton Correctional facility (mail, fax, or email), not to ODRC central mail processing.
