Fill out every field on the visiting application. Oregon DOC returns incomplete applications, which delays the whole process. Each person who wants to visit needs to submit their own application - even if you're in the same household or traveling together.

Watch for returns: If anything is missing, the application can be returned instead of processed. Double-check the form before you send it.

  1. Complete the visiting application - Make sure all requested fields are filled in before you submit.
  2. Mail your completed application - Send it to: Visiting Services Unit, 3723 Fairview Industrial Dr SE, STE 200, Salem, OR 97302.

Submitting your application doesn't mean you're approved. Oregon DOC also notes that the person you want to visit can refuse your request, even if you're otherwise eligible.

  1. Read your denial letter carefully - The 60-day window is based on the date of that letter.
  2. Request an administrative review - Your request must be received by Visiting Services within 60 days of the date of the visiting application denial letter.
  3. Send it early - Aim to get it in well before the deadline so it arrives on time.

Deadline matters: Visiting Services must receive your administrative review request within 60 days of the denial letter date. Mail early and keep proof of when you sent it.

Once the administrative review is decided, that's it - the decision is final. Oregon DOC states the Director's designee's decision isn't subject to further review. Treat your administrative review request as your one shot at reconsideration.
