What Happens If Your Oregon Visiting Application Is Denied (and How to Appeal)

If your visiting application is denied, you can request an administrative review through Oregon DOC using the Request for Administrative Review form (CD 1594). Either you or the adult in custody (AIC) can fill it out. The review looks at the information used to deny the application, including cases where the denial was based on incorrect information or an error in determining eligibility. Fill the form out completely and include any details you want considered.

1. Find the denial letter date: The 60-day window is counted from the date on the denial letter, and Visiting Services has to receive your request within that time.
2. Gather what supports your request: Collect the documents you want reviewed (for example, corrected information or updated official records related to eligibility).
3. Complete CD 1594 fully: Fill out every section and clearly explain why you are requesting the review, including all information you want considered.
4. Mail it early enough to arrive on time: The requirement is “received within 60 days,” so send it with enough time for delivery.

What to Include Where Send

• ✓ Any documents you want Visiting Services to consider as part of the review
• ✓ Documentation showing the denial was based on incorrect information, or that an error was made in determining ineligibility
• ✓ Updated official records showing a change in circumstances that impacts eligibility (if that is the reason you are asking for review)

Once the administrative review is decided, the decision of the Director's designee is final. There is no further review after that.
