What to Know Before You Fill Out the Oregon Visiting Application (CD 50) for Eastern Oregon Corrections Institution

Every person who wants to visit someone at Eastern Oregon Corrections Institution needs to submit their own CD 50. Oregon DOC requires one application per visitor, so don't try to add multiple people onto a single form.

Required Fields Checklist

  • Fill in every blank on the CD 50 (the form states that all information is required).
  • Provide the AIC’s name and SID number where requested.
  • Complete personal details shown on the form, including date of birth, gender, and driver license state and number.
  • If the visitor is a minor, complete the minor section, including the custodial parent or legal guardian’s name, address, and phone number, plus the DHS case questions if they apply.
  • If you also plan to write letters, address the envelope with the AIC’s exact name as it appears in Oregon DOC records (no nicknames), the AIC’s SID number, the correct facility name and complete address, and your return address with your full name and address.

Reminder: The CD 50 says all information is required. If anything is left incomplete, the application will be returned.

Even after you submit the CD 50, the adult in custody (AIC) can refuse a visiting request. If something seems off between what you expected and what actually happens with your request, double-check that you listed the AIC's information exactly as Oregon DOC has it on file.
