How to Get a Copy of an Accident Report from Floyd County Sheriff’s Office

The Floyd County Sheriff's Office only releases accident reports to people directly connected to the crash - involved parties, victims, or insurance agencies listed on the report.

Each copy of an accident report costs $6.00. If you think you’ll need more than one copy (for an insurance claim, employer, or your own records), plan your payment accordingly.

In Person

  • Copy of the involved person’s driver’s license
  • $6.00 payment (cash, personal check, cashier’s check, or money order)

The fee is $6.00 per copy, even when you request the report in person.

Mail

  • Copy of the involved party’s driver’s license
  • A written request
  • $6.00 per copy (pay by cash, personal check, cashier’s check, or money order)
  • A self-addressed stamped envelope

Note: For mailed requests, out-of-county personal checks will not be accepted.

Looking for an incident or offense report instead? That's a separate process. Only closed cases are releasable, and you'll need to submit an open records request to the Civil & Criminal Division at: Floyd County Sheriff's Department, Attn: Civil & Criminal Division, P.O. Box 152, Prestonsburg, KY 41653.

Questions about accident reports? Call 606-886-6711 before you send anything in.

If the crash wasn't investigated by a police officer, you may need to complete and mail a Civilian Accident Report. Call 606-886-6711 for guidance on what to do next - this helps you avoid sending the wrong items. For mail requests, the Sheriff's Office requires a copy of the involved party's driver's license, a written request, payment (cash, personal check, cashier's check, or money order), and a self-addressed stamped envelope.
