MaCI doesn't allow visitors under five years old. If your child hasn't reached that age yet, you'll need to arrange for another adult to visit without them - or wait until the child turns five.

For children who can visit, think carefully about who you list on the Authorization for Minor Child Visitation form. The child can only visit with the people named on that form. If a grandparent, the other parent, or another caregiver might bring the child at some point, add them from the start.

Start by downloading the Authorization for Minor Child Visitation Form from the facility's website. Use the official form - it's what the institution expects when approving minor visits.

The form must be notarized before you submit it. Build in time to sign it in front of a notary and get the stamp before your planned visit.

  • Copy of the child’s birth certificate and/or custody papers (enclosed with the form)

Note: The authorization form includes your acknowledgment that the child(ren) may be searched by employees of the institution.

Need to add a new child, change who can bring them, or remove someone from the form? You'll have to complete a brand new Authorization for Minor Child Visitation form. Updates aren't allowed on the original.

This authorization doesn't last forever - even if nothing changes. The form expires one year from the date MaCI receives it. Mark your calendar and renew it annually to avoid visit delays.
