If a child will be visiting someone incarcerated for a sex offense, you must also include the Acknowledgement of Minor Child Visitation with a Sex Offender (DRC2296). The application won't be reviewed without it.

Note: Some situations - like visiting during reception - have specific requirements. Submit your full packet (DRC2096, signed DRC2554, signed DRC2274, and ID copy) along with any special-case documents upfront.

Submit your packet by mail, email, fax, or in person. Send everything together so it's processed as one request.

Note: Visitation isn’t granted until your application has been processed and you’ve been approved and notified.
