Note: The Superintendent can deny a visit even if your background check has been approved.

Contact the facility where the offender is located to confirm your background check approval and schedule a visit. Do this before making travel plans. The last thing you want is to show up and find out you're not cleared or not on the schedule.

You'll need to complete a visiting request form with personal information, including your date of birth, social security number, race, sex, and place of birth. Gather all of that before you start. It'll help you fill things out quickly and avoid delays from missing details.

Steps to Follow

  • Contact the facility where the offender is located to confirm your background check approval and schedule your visit.
  • Complete the visiting request form with your date of birth.
  • Provide your social security number on the visiting request form.

Note: Even after your paperwork is in and your background check is approved, the Superintendent can still deny the visit.

The visiting request form includes identity fields beyond your date of birth. You'll also be asked for your race, sex, and place of birth. Not sure how to complete a field? Ask the facility directly so your request doesn't get held up over a formatting issue.
