Planning to visit someone at Ohio Reformatory? You'll need to submit a completed Visitor Application (DRC-2096). This is especially true if the person you're visiting is still in reception. For reception visits, adults turn in the completed application at their first visit - you won't be approved until the application process wraps up.
Note: You can send in an application ahead of time, but you can’t visit until it’s processed and you’ve been approved and notified.
The paperwork is a small packet, not just one page. Along with the Visitor Application (DRC-2096), you'll sign the Declaration of Understanding (DRC-2554) and the General Visiting Instructions (DRC-2274). For reception visits, bring all signed forms to your first visit - have everything filled out and ready to hand over.
