Bringing a Child to Visit at Ohio Reformatory: Minor Visitor Application & Notarized Authorization Explained
Bringing a child to visit means some paperwork upfront. Know which form you need—and when notarized permission applies—and you won't get turned away at the door.
To bring a child, you'll need to complete a Minor Visitor Application (DRC2238). Only the child's custodial parent or legal guardian can sign - not just any adult who plans to accompany them. Don't wait until visiting day. The DRC2238 and supporting identity/custody documents must be submitted and approved before the minor's first visit.
Along with the DRC2238, you'll need documents proving the child's identity and who has custody. A birth certificate works for identity; include change-of-custody documents if they apply. Gather these early - approval must happen before the first visit. Once verified, the child appears as an
Note: A visitor listed as an “approved minor” in DOTS Portal can’t deposit money into the incarcerated person’s account, and can’t conduct video visits without a parent or legal guardian.
If someone other than you (the custodial parent or legal guardian) is bringing the child, you'll need one more form: a notarized Authorization for Minor Child Visitation (DRC4371). This isn't a one-and-done document. The notarized DRC4371 must be presented at every visit, along with documentation naming the custodial parent or legal guardian.
Warning: The incarcerated parent cannot be the authorizing signature on the notarized DRC4371.
Practical Checklist Visiting Day
- ✓ Proof the child has been submitted/approved as a minor visitor before the first visit (once verified, they’re listed as an “approved minor” in DOTS Portal)
- ✓ Minor Visitor Application (DRC2238), completed and signed by the custodial parent or legal guardian
- ✓ Birth certificate and/or change-of-custody documents to verify identity/custody
- ✓ Notarized Authorization for Minor Child Visitation (DRC4371) if the custodial parent/legal guardian is not accompanying the child (bring it every visit)
- ✓ Documentation naming the custodial parent or legal guardian (to present with the notarized DRC4371 at the visit)
