How to Get Approved to Visit Someone at Ohio Reformatory: Complete Application Checklist
Getting approved to visit at Ohio Reformatory comes down to paperwork — but one missing item can delay everything. Use this checklist to submit the right forms, include a clear ID copy, and know when to follow up so you can actually get on the schedule.
Planning to visit someone while they're in reception? Apply right away. Adults must submit a complete Visitor Application (DRC2096) at their first visit, so have your paperwork ready before you arrive.
- ✓ Declaration of Understanding (DRC2554) - sign it and submit it with your visiting application.
- ✓ ODRC General Visiting Instructions (DRC2274) - sign it and submit it with your visiting application.
- ✓ Access to these forms - ODRC requires that DRC2274 and DRC2554 be made available to staff, incarcerated people, and visitors (so you should be able to get and review them as part of the process).
Don't submit without your ID. A legible copy of your photo ID must accompany the Adult Visitor Application (DRC-2096). If staff can't read it clearly, your approval gets delayed.
Clarity matters on your ID copy. Make sure the photo, name, and expiration date are easy to read, and that nothing gets cut off at the edges. Taking a phone photo instead of a photocopy? Use bright lighting and avoid glare - washed-out text will slow things down.
Good to know: You’re not required to provide proof of address with your visiting application.
How to Submit and Staff Checks
- ✓ Mail - you can send the visiting application through the U.S. Mail.
- ✓ Email - you can email the application.
- ✓ Fax - you can fax the application.
- ✓ Drop off in person - you can drop the application off in person.
- ✓ Include your ID copy - you must present a copy of your bona fide identification before you can be approved.
Once your paperwork arrives, unit staff run background checks. They search DOTS Portal screens (NAMS1, SCOPE, VNAMS, and VADDS) for past restrictions and visitation history. They also review the incarcerated person's PSI or indictment for co-defendant or victim status, then document everything in DOTS/OnBase.
Two forms you sign - the ODRC General Visiting Instructions (DRC2274) and the Declaration of Understanding (DRC2554) - aren't just for your records. ODRC requires these to be available to staff, incarcerated people, and visitors, which is why they come up throughout the approval process.

Note: Visitation won’t be granted until your application has been processed and you’ve been approved and notified.
