For mental-health information about a female inmate, fax it to Mental Health Services at 323-415-1893. This line is open Monday through Friday, 7:30 AM to 4:30 PM, except holidays.

Need to send medical or mental-health information outside those hours? Use the after-hours line: fax Correctional Health Services at 213-217-4850. This covers after hours, weekends, and holidays.

Important: Do not include anything about pending or upcoming charges in your fax. Keep it strictly to medical information.

  1. Get the right form. Use the Inmate Medication Information Form from the LASD website (English or Spanish).
  2. Print and complete the form. Fill it out fully so staff have clear medication information.
  3. Add the required identifiers at the top. Start your fax with your loved one’s full legal name, date of birth, booking number, and location.
  4. Choose the correct fax number based on gender and time. For men, fax Mental Health Services at 213-972-4002 (Monday to Friday, 6:00 AM to 5:00 PM, except holidays). For women, fax Mental Health Services at 323-415-1893 (Monday to Friday, 7:30 AM to 4:30 PM, except holidays).
