How to Fax Medication or Mental-Health Information for Someone at Twin Towers (LASD)
LASD has an Inmate Medication Information Form for sharing medication and mental-health details about someone at Twin Towers. You can download it in English or Spanish from the LASD website. Print it, fill it out completely, then fax it using the numbers below.
Start your fax with clear identifying information so staff can match it to the right person. At the top, include their full legal name, date of birth, booking number, and current location. Sending multiple pages? Put that same header on every page - it keeps everything together in the correct file.

For someone in the women's system, fax Mental Health Services at 323-415-1893. Hours are Monday through Friday, 7:30 AM to 4:30 PM (closed holidays).
