Visiting Application Checklist for USCF: What to Include and When to Renew
Getting approved to visit at the Utah State Correctional Facility (USCF) comes down to one thing: submitting a complete, accurate application the first time. Then you just need to keep it current so your approval doesn't lapse.
The USCF visiting application isn't just basic paperwork. When you sign, you're certifying that everything you provided is accurate and that you'll follow the visiting rules and staff directions. Review every line before signing - the form makes clear that falsifying any information means automatic denial.
You'll need details for both yourself and the incarcerated person. For them: full name, offender number, housing assignment, and the date of request. For you: full name (including maiden name if applicable), date of birth, address, driver's license or ID number with the issuing state, and your relationship to the inmate. Applying for children too? The forms let you submit applications for yourself and minors together.

- ✓ Adults and minors age 16+: Color photo of a state-issued ID
- ✓ Minors: Color copy of birth certificate(s)
- ✓ If you’re including minors on your paperwork: Use the application option that accommodates adults and minors
- ✓ Non-parent guardians: Proof of guardianship for each minor listed
- ✓ Adult sibling(s) or grandparent(s) bringing a minor: Proof of relationship to each minor
- ✓ Adult sibling(s) or grandparent(s) bringing a minor: Written, notarized permission letter from each minor’s parent/legal guardian
- ✓ If you have a medical device: A doctor’s note verifying the medical device
Plan ahead. USCF visitor applications typically take 6–8 weeks to process. If you're hoping to visit for a birthday, holiday, or milestone, submit your application early so you're not left waiting.
Tip: If you want to be notified whether your application is approved or denied, include a self-addressed stamped envelope with your application.
Approval isn't permanent. All visitors must update their application every year. Mark your calendar for 12 months after your approval date - submitting a renewal is your responsibility, and missing it means losing your visiting privileges.
If your address changes after you’re approved, don’t wait until your next renewal. You must notify visiting staff in writing within 10 days of your new address. Treat this like a deadline - missing it can put your visiting privileges at risk.
