How to Stop an Incarcerated Person From Contacting You in Illinois (Mail Stop & Phone Restriction)

You'll need the Mail Stop and Phone Restriction Request form, available in both English and Spanish. The form asks for your status - whether you're a victim, witness, victim's next of kin, guardian of a victim, or concerned citizen. You'll also need to fill in the Warden and correctional center where the person is housed, since the request goes to that facility.

After completing the form, email it to DOC.VictimServices@illinois.gov. You can also mail it to the IDOC Victim Services Unit - or directly to the facility Warden, depending on the instructions on the form.

  • Fill out every section of the Mail Stop and Phone Restriction Request form, including your requester status (victim, witness, concerned citizen, etc.)
  • Complete the facility-related fields on the form (the correctional center and Warden information)
  • Submit the form by email to DOC.VictimServices@illinois.gov or mail the completed form to the IDOC Victim Services Unit (as directed on the form)
  • Include any supporting documentation if the form instructions ask for it
