Your first visit to Warren Correctional Institution goes much smoother when your paperwork is right the first time. Use this checklist to submit the required forms and ID copy, answer every question honestly, and complete the separate registration step needed to reserve your visit.

Before Warren can approve you to visit, you need to complete three things: the Visiting Application (DRC-2096), the Declaration of Understanding (DRC-2554), and the General Visiting Instructions (DRC-2274). Think of these as a packet - if any piece is missing, your application won't move forward.

Tip: Complete the DRC-2096 and sign both the DRC-2554 and DRC-2274 before submitting anything. All three must be done for approval.

Include a legible copy of your photo ID with your application. If the copy is too dark, blurry, or cut off, staff can't verify it - and that slows everything down.

Approval doesn't mean you're done with ID requirements. You'll register and show identification at check-in every visit. Bring your ID each time - prior approval alone won't get you through the front desk.

Warning: If you leave any question unanswered or falsify any answer on the adult visitor application (DRC-2096), the application will be disapproved.

Getting approved is a big step, but it's not the last one. After approval, you still need to register with GTL/ViaPath (the GTL VisitMe system) before you can book a visit. Approval puts you on the allowed visitor list. Registration is what lets you actually reserve a time slot.

  1. Enter the incarcerated person’s details - provide their last name and ID number so the reservation is tied to the right person.
  2. List every adult who will attend - include the first and last names of all adult visitors.
  3. Add minors the right way - if a minor is related to the incarcerated person, provide the minor’s first name and the relationship; if not related, list the minor’s age only.

Reminder: Once you're approved and registered to reserve visits, you'll still need to register at check-in each time and show valid identification.
