To visit at Warren Correctional Institution, you need to complete the Ohio Department of Rehabilitation and Correction visitor paperwork. Approval isn't automatic. You won't be able to visit until your application has been processed. The core document is the Visiting Application (DRC-2096). You'll also need to sign two acknowledgement forms: the Declaration of Understanding (DRC-2554) and the General Visiting Instructions (DRC-2274). All three are part of the approval process, and a missing signature on any of them can slow things down.

  • Visiting Application (DRC-2096) - the main application that must be completed before you can be approved to visit.
  • Declaration of Understanding (DRC-2554) - your signed acknowledgement of the visitation rules and expectations.
  • General Visiting Instructions (DRC-2274) - your signed confirmation that you have read and will follow the visiting instructions.

Your application needs to include a legible copy of your photo ID. "Legible" means staff can clearly read the details and see the photo. Use a clean copy that isn't blurry, cut off, or too dark. Once you're approved, bring bona fide identification with you every time you visit. You'll need to register and show bona fide identification at check-in for each visit.

Tip: When copying your ID, make sure your name, ID number, and expiration date are all easy to read. If any part is hard to see, your application could get held up during review.

Warning: If you leave any question unanswered or falsify any answer on the adult visitor application, the application will be disapproved.

  1. Wait for approval - visitation starts only after your application has been processed and approved.
  2. Register with GTL/ViaPath - once you are approved, you must register in their system.
  3. Make a visit reservation - after you register, you can schedule your visit reservation through GTL/ViaPath.

Getting approved is a big step, but not the last one. You still need to complete the GTL/ViaPath registration and make a reservation before you show up. On the day of your visit, bring bona fide identification. Warren Correctional requires visitors to register and show bona fide identification at check-in every time.
