TDOC views visitation as part of rehabilitation and wants to encourage family involvement. That's why there's a formal approval process - eligible people can visit, but only after the facility has your application on file and you've been cleared through TDOC's guidelines.

Step1 Fill Form

  • Fill out every section of the CR-2152 (don’t leave blanks)
  • Use a physical address for the applicant address (P.O. Boxes aren’t allowed)
  • Attach a recent photograph in the lower left-hand corner of the form

Two things slow applications down: incomplete sections and missing photos. TDOC also won't accept a P.O. Box as your address on the CR-2152 - use a street address where you can reliably receive mail.

How to Get Approved to Visit Someone at Debra K. Johnson Rehab Center

Every applicant goes through an NCIC background check, so expect your information to be reviewed before approval. Approval isn't permanent either - TDOC requires visitation applications to be updated every 24 months. If you want to keep visiting, plan to reapply on that cycle.

  • A current, recent photograph (attached in the lower left-hand corner, not a photocopy)
  • A self-addressed stamped envelope if you want a visitors’ handbook sent to you
  • A reminder on your calendar for the 24-month update requirement
  1. Prepare your envelope - Put the completed CR-2152 inside after you’ve confirmed every section is filled out and the photo is attached.
  2. Address it to the right office - Use: Associate Warden of Security (or Deputy Superintendent), plus the prison name and prison address.
  3. Mail it to the facility - not to the person you’re visiting - The form needs to go directly to the Associate Warden of Security/Deputy Superintendent for processing.

Note: Forms mailed directly to the offender won't be accepted.

After the facility receives your CR-2152, TDOC says applications should be approved or denied within 30 days. Use that as your baseline for planning. If you're mailing it in, factor in delivery time plus that processing window before expecting to visit.

You generally can't visit until your application is approved. There's one exception: immediate family of newly committed inmates may get temporary visiting privileges while the application is pending - but only for up to 60 days from the inmate's intake date.

Applications with falsified information will be denied. The penalty is steep: if you submit false information, you can't reapply for at least six months. Even a small untruth can delay your visits significantly.

Note: Double-check every answer on your CR-2152 before mailing - falsified information triggers a six-month wait to reapply.

  • A recent photo is attached in the lower left-hand corner
  • You included a self-addressed stamped envelope if you want a visitors’ handbook mailed to you
