How to Get Approved to Visit Someone at Allen‑Oakwood: Complete Checklist
Getting approved to visit at Allen-Oakwood comes down to three things: the right paperwork, proper signatures, and a clear copy of your ID. Use the checklist below to make sure your application goes through the first time.
You will need to complete the Visiting Application (DRC-2096), sign the Declaration of Understanding (DRC-2554), and sign the General Visiting Instructions (DRC-2274). Include a copy of your bona fide identification with the application package.
Warning: Include a legible copy of your photo ID with your DRC-2096. Answer every question fully and truthfully. Leaving anything blank or falsifying answers will get your application denied.
Your photo ID copy needs to be legible. That means staff can clearly read all the details, and nothing is cut off, blurry, or too dark. A hard-to-read copy can delay processing or cause problems with your application.
Note: All visitors are subject to search when entering the facility.
You will also need to sign two additional documents: the Declaration of Understanding (DRC-2554) and the General Visiting Instructions (DRC-2274). These go with your application and ID copy. By signing, you confirm that you understand the visiting rules you will need to follow once approved.
