Getting Approved to Visit Menard: How the Prospective Visitor Interview (PVI) Works

Every adult visitor (18 and older) at Menard must complete the Prospective Visitor's Interview form, DOC 0148. The only exceptions are legal visitors and government officials.

Completing the PVI is just one piece of the puzzle. You also need to be on the incarcerated person's approved visitation list before you can actually visit. Legal visitors and government officials are the only exceptions.

The incarcerated person is responsible for arranging visits and letting you know your status. Not sure if you're approved? Ask them directly - it's the fastest way to find out.

You can download the DOC 0148 Prospective Visitor's Interview form from the Illinois Department of Corrections (IDOC) website. It's available in English and Spanish. Filling it out before your first trip saves time at check-in.

Haven't completed the DOC 0148 yet? Plan on doing the PVI during your first visit. The only exception is if you've already received prior approval through the electronic PVI process.

The PVI isn't just a form drop-off. IDOC staff may interview you or request background information to determine whether you pose a safety or security risk. If they ask follow-up questions, it's usually because they need more clarity before making a decision. Answer directly and consistently - it helps avoid delays.

Tips Avoid Delays

• ✓ Download the DOC 0148 (Prospective Visitor’s Interview) ahead of time from the IDOC website (English or Spanish) and complete it before you go.
• ✓ If you’re expecting approval through the electronic PVI process, confirm that prior electronic approval has been received so you don’t get surprised at your first visit.
• ✓ If you don’t have prior electronic approval, plan time on your first visit to complete the PVI step at the facility.
