Every potential visitor needs to complete three forms: the Visiting Application (DRC2096), the Declaration of Understanding (DRC2554), and the General Visiting Instructions (DRC2274). You'll also submit a copy of your government-issued ID as part of the process. Make sure the information on your forms matches your ID exactly.

Documents to Include

  • Include a legible copy of your photo ID with your visitation application.
  • If you’re currently on probation or parole, include a letter from your probation/parole officer granting you permission to visit.

You can submit your application by U.S. Mail, email, fax, or in person. Whichever method you choose, plan for processing time. Turning in paperwork doesn't mean you're approved - you can only visit after the application has been reviewed and you've received official notification.
